The Home and Community Based Services (HCBS) standards under Minnesota Statutes, chapter 245 D, are part of a larger HCBS Waiver Provider Standards initiative to improve the dignity, health, and independence of the people we serve.

Housing Stabilization Services

Housing Stabilization Services is a newer Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and maintain stable housing.

Housing Stabilization Services

Consultation

For individuals without a waiver, targeted case management, or a MSHO/MSC+ care coordinator but would like HSS, FHHC's housing consultant can complete a Housing Focused Person-Centered Plan to enroll in HSS. We provide assistance with gathering necessary documentation, and develop a plan of housing needs and goals for their chosen provider.

Transitioning

We assist clients with housing research, based on their personal needs/wants in housing. Our staff works alongside clients to complete housing applications, gather necessary documents, find funding for deposits, understand a lease, plan/organize a move, and more. Staff can assist with communicating to property managers/landlords throughout the process, and we aim to maintain a person-centered focus in our approach to housing.

Sustaining

Once a client secures and moves into stable housing, our staff continues to work with clients to ensure they maintain their housing. We assist with communicating with property management, identify problems that could affect their housing, identify resources to assist if there's a risk of losing housing, and general support during a client's tenancy.
