245 D Waivered Services

The Home and Community Based Services (HCBS) standards under Minnesota Statutes, chapter 245 D, are part of a larger HCBS Waiver Provider Standards initiative to improve the dignity, health, and independence of the people we serve.

  • Individual Home Support with Training

  • Individual Home Supports with Family Training

  • Individual Home Support without Training

Housing Stabilization Services

Housing Stabilization Services is a newer Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors finding and keep housing.
